Media Release Form

I hereby grant ID43 Creative authorization and permission to use, reproduce or publish, without payment or consideration, any photographs or videos and/or any other media representations, written statements,  interviews and quote, that I may make or take on, during, or in relation to a photo or video shoot, including online photographs or videos, website entries, brochures, press releases, advertisements and other communications that may or may not be presented to the public.

I understand and agree that all such these materials may be hereby used ID43 Creative in its sole discretion without my consent or need for approval and that I will share copies of any and all materials upon request by ID43 Creative.

I hereby hold harmless and release and forever discharge ID43 Creative and all persons functioning under the Company’s permission or authority, from all claims, demands, and causes of action which I or my employer may have by reason of this authorization.

I am over 18 years of age and have the right to contract in my own name. If under 18, my legal guardian has contracted on my behalf.  I have read this release before signing below and I fully understand the contents , meaning, and impact of this release.

Specific to photography and video: I hereby irrevocably authorize ID43 Creative to edit, alter, copy, exhibit, publish or distribute any photo or video for purposes of publicizing ID43 Creative, their partners, or for any other lawful purpose.

In addition, I waive the right to inspect or approve the finished product, including written or electronic copy. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph or videography.
